North Korea has unsuccessfully test-fired another missile, just hours after it was warned not to at a Security Council meeting in New York.

Rex Tillerson, the US secretary of state, told the UN body there would be "catastrophic consequences" unless the world came together to stop North Korea. And he refused to rule out US military action.

Al Jazeera's Rosiland Jordan reports from the UN.
